Choice of foam for berth cushions?

It's time I replaced the foam in the master berth and wondering what to use.
These cushions are used only for sleeping on but get a lot of use. My wife moans they hurt her back.
I've replaced the covers a couple of times but the foam is still the original and not in great shape.
Any recommendations on which grade of foam, hardness etc?
Should I be building the foam out of layers of different foam? i.e. 3 inches normal foam and 1 inch of memory foam. (Overall thickness is 4".)
I was considering using www.gbfoam.co.uk as they are not too far from me.

You don't say if your wife finds them too hard or too soft. It's a personal thing - I like soft because my hips complain. I have used a memory foam topper in the past and found it very good but the boss found it too hot.

My queen size bed is 3 layer foam made in 3 sections so we could get it in.

My seating is 2 layers medium density on top and reconstituted under.
